Rehabilitation & Community Re-Integration Services
Individuals who experience severe & persistent mental illnesses are often very vulnerable to daily stressors. Individuals with these brain disorders often experience difficulty with learning, socialization and activities of daily living. This frequently leads to increased anxiety, decompensation, hospitalization, and recurring symptomology. The Goal of the program is to partner with individuals who desire to make positive changes in their lives and enhance education, work and social integration in their community.
Rehabilitation & Community Re-Integration is designed to teach psycho-social skills that will assist persons served in successfully navigating their complex communities.
Treatment is designed to help acquire the knowledge and skills necessary to take personal responsibility for health and to support efforts to proceed with life beyond illness. The program is self-directed and focuses on decreasing hospitalization by increasing independence and self-esteem through learning and applying effective behaviors in social situations. The program is based on a strengths/recovery model that utilizes the participant's abilities and strengths to encourage and motivate them to obtain successful skills.
